All individuals who desire one of the Committee Selection exemptions into the 2009 HealthONE Colorado Open Championship MUST complete this form. The Tournament Committee of the Colorado Open Golf Foundation will be responsible for the selection of these exemptions. The Committee will evaluate all applications submitted prior to June 28th; requests submitted following this date shall NOT be considered for any reason. The Committee will ONLY review information submitted on this form, so please do not submit any additional information or materials.
