| At-Large Exemption Application |
All individuals who desire one of the at-large exemption positions into the 2008 HealthONE Colorado Open Championship MUST complete this form. The Tournament Committee of the Colorado Open Golf Foundation will be responsible for the selection of up to ten (10) at-large exemptions. The selection of the at-large exemptions shall be made by the Committee at the selection meeting on Thursday, June 26, 2008 and announced as soon as practical following the meeting. The Committee will evaluate all applications submitted prior to the entry deadline; requests submitted following the deadline date shall NOT be considered for any reason. The Committee will ONLY review information submitted on this form, so please do not submit any additional information or materials.

| Caddies & Carts |
Caddies: Caddies are allowed; however, the Colorado Open Golf Foundation is not responsible for furnishing caddies and recommends that players bring their own. Players are not required to employ a caddie for the Championship and may carry their own bag.
Carts for Qualifying Rounds: Players may ride in the Qualifying Rounds ONLY--not in the Championship. Cart fees are payable at date of qualifying to the host site golf shop.
Carts for Championship: PROHIBITED—All players and caddies must walk.
Push Carts & Pull Carts: Permitted--a limited number of pull carts are available for rent on a first come first served basis from Green Valley Ranch Golf Club.
Disabled Participants & Spectators: The Committee reserves the right to waive the no-cart rule for anyone with a disability as defined under the Americans with Disabilities Act (ADA). All such requests and required supporting information must be submitted in writing with the entry application by the deadline. Requests due to temporary injuries or impairments that do not qualify as a "disability" under the ADA will not be granted. Cart availability for spectators is not guaranteed and riders must provide their handicap parking permit or like written verification of their disability to be eligible to rent a cart. The per rider fee is due to Green Valley Ranch Golf Club. |
| Exemption Categories & Listing |
Exemption Eligibility
Players in the following categories shall be exempt into the Championship provided that a completed entry, with payment, is received prior to the stated deadline, and he or she is otherwise eligible. Exemptions reserved for amateurs will be granted only to players who remain amateurs through the duration of the Championship. An amateur who accepts an exemption from qualifying becomes ineligible to compete in the Championship if he or she becomes a professional before or during the play of the Championship.
Exemption Categories
Exemptions from qualifying are available in the following categories:
- All former Colorado Open Champions
- Top 30 plus ties (amateur and professional) from the 2007 HealthONE Colorado Open Championship
- The Colorado Section of the PGA has 27 exemptions to use at their discretion.
- The Colorado Golf Association has 20 exemptions to be issued at their discretion to the top Colorado amateurs and in accordance with the amateur eligibility requirements listed above.
- Championship host site professional
- The top returning player (as determined by 2007-08 end of season scoring average) on the men’s golf team from the following local NCAA Division I programs: Air Force, University of Colorado, Colorado State University, University of Denver, University of Northern Colorado and the University of Wyoming. Head Coaches will receive invitation to offer to the player. If the top player is already exempt under another category, this exemption may be used by the school's #2 player.
- Sponsor Selection—up to 3 exemptions will be granted upon committee decision.
- At-Large Exemptions—up to 10 exemptions will be granted by committee based upon playing resume. Individuals who desire one of these exemptions MUST submit an At-Large Exemption Application to the Colorado Open Golf Foundation office by the entry deadline--no other information will be reviewed. Applications may be obtained from the above link or from the Colorado Open Golf Foundation office. Players applying for an at-large exemption are encouraged to submit an entry form complete with payment by the deadline in order to secure a position in one of the qualifying rounds in the event that they are NOT selected for an exemption.

| Field Composition |
The entry forms provided by the Colorado Open Golf Foundation (including on-line forms) or photocopies of these entry forms must be used for entry by all players, including those that are exempt. Forms may be obtained from the Colorado Open Golf Foundation office or website (www.ColoradoOpen.com). Only one entry may be filed per player. The Colorado Open Golf Foundation reserves the right to disqualify anyone who submits more than one entry. Entries received before the deadline and after the field is full will be placed on standby in order of receipt at the Colorado Open Golf Foundation office. Entries received after the deadline are not acceptable and will be automatically rejected. Entries should be submitted early to allow ample time for delay or error in transmission. The risk of delay or error in transmission lies solely with the entrant. The Colorado Open Golf Foundation shall have no liability with respect to any such delay or error and the consequences therefrom, including rejection of the entry. Confirmation of entry will be mailed or emailed after the entry deadline.
Championship Field Limit: The field is limited to 156 players comprised of place-winners in the qualifying rounds and those exempt. At least 60 spots in the Championship will be awarded to the top finishers from the qualifying rounds.
Qualifying Round Field Limits: Entries are taken on a first-come, first-accepted basis until all four qualifying round fields are filled at 84 players in each. Amateur applications must undergo verification of Handicap Index before final acceptance. |

Qualifying: Qualifying round entrants may schedule non-complimentary practice rounds by calling the host site directly: Eagle Ranch GC at 970-328-2882; Legacy Ridge GC at 303-438-8997; or Springs Ranch GC at 719-573-4863.

| Purse & Prize Payout |
The purse is $125,000 (which includes up to $5,000 in amateur merchandise prizes). Prize money will be paid to all players that make the 36 hole cut and complete the final round of play. Amateurs will receive their payment in the form of merchandise gift certificates redeemable at Green Valley Ranch Golf Club. All professionals that complete the final round of play receive a minimum of $300.
The Champion Receives
- $23,000 and trophy
- An exemption from qualifying for future Colorado Open Championships
The Low Amateur Receives
- $750 gift certificate
- The Dave Nelson Trophy
Players must complete the final round of play to receive prize money.

Play Date Assignment
Each Qualifying Round has a field limit of 84 players.The fields are filled on a first-come, first-accepted basis until full. The number of Championship qualifying spots for each qualifier will be determined by the percentage of players in a qualifier in relation to the total number trying to qualify. If a qualifying time has more players than can be accommodated some players may be transferred to another date and time than the original requested time and date. Transfer will be based on the date of receipt of entry application. No qualifying time is final until after entries close.
Alternates
Two players from each qualifier with the lowest scores not qualifying will comprise the Championship alternate list. The eight alternates will be ranked in order based on a blind draw of the four Qualifying Rounds, (i.e. 1st and 5th from the first round drawn, the 2nd and 6th from the second round drawn, etc.). If an exempt player withdraws from the Championship, alternates will be admitted in rank order. In the event that a qualifier participant withdraws from the Championship, the top alternate from that same qualifying site will fill the void spot.
Tie Breaking
A sudden death playoff will decide ties for the last qualifying position as well as the two alternates. The playoff will be conducted immediately following completion of the Qualifying Round at a hole determined by the Committee.
Notice of Withdrawal
If a player withdraws from a Qualifying Round, they must notify the Official in charge of the event promptly in order to insure a standby player has a fair opportunity to play. Failure to comply may result in disqualification from future Colorado Open Golf Foundation events. |
| Refunds & Withdrawals |
All exempt players must pay the full entry fee at time of entry.
Refunds: A full refund of the entry fee will be issued to all entrants not admitted into an event. There is a $45.00 service charge for ALL withdrawals prior to the entry deadline. Players who contact the Colorado Open Golf Foundation office to withdraw after the deadline date, but at least by 5:00 P.M. MDT one week prior to the first day of the event, will receive a 50% refund of their entry fee. Unless deemed appropriate by the Committee, players are not eligible for refunds after this cutoff. There are no refunds for non-qualifiers and no refunds will be issued for any reason after the start of the tournament. All refunds are issued to the person listed on the entry form. Please allow up to two weeks for processing refund checks.
Withdrawals: If a player must withdraw from a Championship or Qualifying Round once it has begun, he or she should notify the Colorado Open Golf Foundation office or tournament site as soon as possible with the reason for the withdrawal. “No-Carding,” “No-Showing,” and withdrawing from an event without a valid excuse is NOT acceptable. All players needing to withdraw from an event must notify a member of the Committee prior to leaving the tournament site. Failure to do so will result in a one-year suspension and loss of any future exemptions from any Colorado Open Golf Foundation event. |
