Title:	At-Large Exemption Application
Detail:	All individuals who desire one of the at-large exemption positions into the 2008 HealthONE Colorado Open Championship MUST complete this form. The Tournament Committee of the Colorado Open Golf Foundation will be responsible for the selection of up to ten (10) at-large exemptions. The selection of the at-large exemptions shall be made by the Committee at the selection meeting on Thursday, June 26, 2008 and announced as soon as practical following the meeting. The Committee will evaluate all applications submitted prior to the entry deadline; requests submitted following the deadline date shall NOT be considered for any reason. The Committee will ONLY review information submitted on this form, so please do not submit any additional information or materials. Application form
